Liver, spleen and pancreatic surgery covers the surgical treatment of liver cysts and masses, hydatid cyst, splenic disease and pancreatic pathology. Splenectomy can be performed laparoscopically in suitable patients. Most benign liver lesions are simply observed; surgery is considered where lesions cause symptoms or are growing.

The liver, spleen and pancreas are the solid organs of the abdomen. Surgery on these organs is among the technically demanding areas of general surgery and requires separate experience.
Liver Conditions
Liver cysts
Simple liver cysts are common, usually cause no symptoms and are observed. For cysts that grow or cause pain or pressure, laparoscopic removal of the cyst roof (deroofing) may be performed.
Liver haemangioma
The most common benign liver tumour, made up of a tangle of blood vessels. It is usually found incidentally and needs no treatment. Surgery is considered only for very large, symptomatic lesions or those at risk of bleeding.
Hydatid cyst
Caused by a parasite (Echinococcus) carried by dogs, this condition is still seen in Turkey. Cysts in the liver can grow and cause pressure, or rupture and lead to serious complications. Medication, percutaneous intervention and surgery are considered according to the stage of disease.
Liver abscess
May be bacterial or amoebic, presenting with fever, right upper abdominal pain and fatigue. Drainage may be needed in addition to antibiotic treatment.
Splenic Surgery
The spleen is an important part of the immune system and of blood cell breakdown. Splenectomy is considered for:
- Blood disorders such as ITP (immune thrombocytopenic purpura)
- Hereditary spherocytosis and similar haemolytic anaemias
- Splenic cysts and tumours
- Traumatic splenic injury
- Marked splenic enlargement (splenomegaly)
In suitable patients laparoscopic splenectomy is preferred, giving less pain and faster recovery than open surgery. Certain vaccinations before surgery are important in reducing the risk of infection afterwards.
Pancreatic Conditions
Acute pancreatitis
Sudden inflammation of the pancreas, most commonly caused by gallstones or alcohol. It presents with severe abdominal pain radiating to the back, nausea and vomiting. Treatment is mainly supportive; where gallstones are the cause, removing the stone and the gallbladder is required.
Pancreatic cysts and tumours
With the spread of imaging, pancreatic cysts are detected more often. Some are observed while those with suspicious features are assessed for surgery. Pancreatic tumours require different surgical approaches according to location.
Adrenal Gland Surgery
Hormone-secreting or suspicious masses in the adrenal gland are removed by laparoscopic adrenalectomy in suitable cases.

Most haemangiomas require no treatment and are monitored with periodic imaging. Surgery is considered only for very large lesions, those causing marked symptoms or those at risk of bleeding.
Will my immunity weaken if my spleen is removed?
After splenectomy the risk of infection with certain bacteria increases. For this reason pneumococcal, meningococcal and Haemophilus vaccines are given before surgery, and patients are advised to seek help early with febrile illness.
Is a pancreatic cyst cancer?
Most pancreatic cysts are benign, but some can be precursors to cancer. The distinction is made according to the type, size and imaging features of the cyst; suspicious cases are assessed for surgery.
